Students who play on any middle school sports team will need a physical exam dated within the past two years uploaded directly onto their FinalForms account in order to participate. FinalForms is managed by BMS Athletic director, Lauren Cass, and coaches. This is a separate program from SNAP Health Portal that the BMS Health office uses. The Health office encourages you to upload your physicals and immunization records yearly.

Students are required to have a dose of Tdap prior to 7th grade per New Hampshire Department of Health and Human Services.
